Our Simulation and Integrative Learning Institute (SAIL) at UIC has a standard evaluation that has a likert scale addressing the goals/objectives/content, facilitator, environment, overall. It also has freetext questions about three things the learner learned that day, something they found to be specifically helpful, and something that can be done differently to improve the program.
